This is the CAVS membership application form. Thank you for your application.
Completing this form is the first step to becoming a member. On receipt of your application we will ask you to share your governing documents with us. We will then consider your application to ensure it complies with the eligibility criteria as listed on our membership web page.
This form has 23 questions and should take about 10 minutes to complete. It is best completed by someone with a governance/ management role within the organisation.
