CUSP – Carmarthenshire United Support Project
CUSP is a third sector collaborative partnership which aims to support people to live well and independently and reduce the impact of disability and ill health.
CUSP targets people aged 18+ at risk of losing independence through poor health or increasing frailty.
Its aim is to prevent their needs from developing further, to slow down deterioration, help them regain independence and provide an effective route to statutory services only when they are needed.
The project aims to connect individuals back to their community and provide practical solutions that support & maximise independence.
CAVS is pivotal to the partnership being a success and provided the secretariat for the continued development of the programme.
