Connectors’ Network

Connectors’ Network is a Network for link workers across the county. We meet informally, approximately 6 weekly basis, to share our news, identify service gaps and co-produce possible solutions.

Social prescribing / community connecting across Wales is gaining traction. As practitioners in Carmarthenshire,  we are passionate to do what we can to make the experience positive, empowering and truly person centred.

To do that we support one another to reduce duplication, better use our limited resources and tap into the wealth of experience and knowledge we possess as a team, across multiple agencies.
